Patient Information

Thank you for choosing our practice! We are committed to the success of your medical treatment and care. Please understand that payment of your bill is part of this treatment and care.

For your convenience, we are inclosing this letter to explain our registration process, financial policy and items that you will need to bring to your appointment.

What Is My Financial Responsibility for Services?
As a courtesy to you, we will bill your insurance company. Insurance companies sometimes us the phrase “usual and customary” when discussing processional and facility fees. The insurance companies set their own “usual and customary” rates based on wide geological area and the fees we charge differ. You will be billed for any applicable copayments, and/or deductibles, and/or exclusions of your insurance contract. We encourage our patients to contact insurance company prior to coverage, and or preauthorization treatments.

We ask that you realize that services are rendered to a person, not an insurance company. Hence, the insurance company is responsible to the patient and the patient is responsible to us. If we do not participate with your insurance company you will be billed for all charges they deem over the “reasonable and customary”.

Our office staff will call your insurance company ahead of time to determine your coinsurance and/or deductible benefits. Payment is collected at the time of service.

How May I Pay?
We accept payment in the form of Cash, Check, Visa, Mastercard and Care Credit. [For more information, please see our Payment Policy.]

No Show and/or Late Appointments
Our office is here to provide a service to you and your family. When a patient is late or no shows for an appointment, it prevents us from seeing and treating another patient in a timely manner. If you are late or are unable to attend the appointment, please give a courtesy call to let us know.
